1. Yes you can get any no. of extensions based on approved I140 from employer A.
2. Yes, she can renew based upon your H1B transfer I797 copy and approved I140 copy.

My RFE was related to experience letter to be shared in GC format on company's letter head. I tried to contact my 2 previous employers, one has given me that letter but other has refused so I converted that email communication in pdf format then it got approved.

There is no timeline on GIT to file H1B extension after document submission as it depends upon their workload and employee's current visa expiration date. In most cases they tried to file in the last 1-2 months before expiration date.

1. Yes correct, they will not contact you if they don't need from your end. In my case they contacted me when 1 month was left to respond on RFE.
2. 87 days for RFE deadline.
3. You can wait for one more month but I advise to file extension based on PERM as it might take 2-4 months to get approval on I 140. My RFE was responded on Dec 16th and got approval on Jan 31st but it depends on case by case.
4. I m not sure.
5. Yes, as you are not sure what RFE is about in first place and when it is going to be responded. More details given in point #2.

I received I140 Approval Email from GIT today.
I140 receipt date - Mar 16 2017.
RFE receive date: Oct 2 2017
RFE responded: Dec 16 2017
I140 Approval Date - Jan 31 2018.
USCIS site still not updated and shows "Case Received".

I would advise to wait for 180 days period after I 140 approval as you never know when the employer's mind gets changed. If that happens you will loose your priority date and you will have to start from scratch.

I wouldn't recommend for premium after RFE, I would rather say to keep following up with GIT team via GSD request or follow up email to file the RFE response as early as possible and wait till June 2018. If the decision doesn't come up then you can go for premium.

The extension can be filed at least 2 week prior to expiry date so that you can get the extension receipt on time. In my case extension was filed on May 25th this year while my expiry date was on June 15th, hope this helps you.

We are not sure about the decision time on RFE response, it might be done in 1 months or it might take >3 months vary on case by case. Mine RFE was responded on Dec 14th and I am still awaiting. As stated by shroh, you can still file for 1 year extension based upon your PERM approval if the perm got approved prior to 365 days based on your max out.
